How to register with the practice, how to get here and what you need to know!

University Students

At Ireland Wood and Horsforth Medical Practice we know that starting university can be a confusing and overwhelming time. We want to make the registration service as easy as possible so that you can seek medical treatment whilst living away from home.
If you are studying at Leeds Trinity University, then you fall within the geographical catchment area of our Horsforth practice, The New Croft Surgery.

How to register as a patient

In order to register with the practice you will need to complete a patient registration form. You will need to bring the completed paperwork along with some photo ID and proof of address into the practice so that we can set you up on our system.
By the end of March 2020, you will be able to register online rather than having to come to the surgery in person, and will be able to provide your identification documents by uploading a photo – you will be able to do this on a smart phone.

Opening Hours – New Croft Surgery

The New Croft Surgery is open Monday – Friday each week. The surgery is closed on bank holidays.
We, and the other practices in the city, close for one afternoon each month in order to provide essential training for staff. This is usually on the last Thursday of each month although it can vary dependent on Leeds Clinical Commissioning Group. There is also an Ireland Wood Surgery Facebook Page, which provides similar information about our Ireland Wood site in Cookridge.

We also close each site bimonthly for staff meetings. These are also held on a Thursday and rotate between the two sites. This means that your surgery will only close once every two months for such meetings. The meetings take approximately 1.5 hours and are advertised on our website and Facebook page. We also display closure information in the surgery.

Tuesday: 8am – 6pm
Wednesday: 7am – 6pm
Thursday: 8am – 6pm
Friday: 7am – 6pm

Opening Hours – Ireland Wood Surgery

Ireland Wood Surgery is open from Monday to Friday each week. There is also an evening and weekend hub service to provide health care to patients outside of regular opening hours.

Monday: 7am – 6pm
Tuesday: 7am – 6pm
Wednesday: 8am – 6pm
Thursday: 8am – 6pm
Friday 8am – 6pm

Opening Hours – Ireland Wood Hub

The Ireland Wood Hub service is managed independently to the surgery, but is hosted at the Ireland Wood Surgery. Any feedback or queries about the hub should be directed to the Ireland Wood Hub and not Ireland Wood and Horsforth Medical Practice.
The Ireland Wood Hub opens regularly throughout the week and on weekends. The Hub also opens on bank holidays. Information about this service is published on our website and practice Facebook accounts.

Monday – Friday: 6pm – 8pm
Saturday 8am – 4pm
Sunday 9am – 1pm

Online Access to book/cancel appointments and order prescriptions

Please ask at the surgery for a form, or click here to be redirected.

You will need to provide a form of ID (driving licence, passport etc.) and then you will be given a pin number and login information about how to access online services.

Vaccinations

Please book in at the surgery as soon as possible if you have not had two MMR vaccinations or your Meningitis ACWY.

First Request for Medication

You mist attend an appointment in person in order to be issued with a repeat prescription for the first time. Please book the appointment in good time to ensure that you do not run out of medication.
