Update on Online Service Access

Thank you once again for your patience in dealing with the issues with online access since our change in clinical system.

We are aware that for a number of patients the problems persist and are doing our best to get through the requests. The vast majority of patients have been very understanding about this, and we would like to thank you for your kindness towards the team.

There are a couple of points that we need to make patient’s aware of in regards to online access:

  • Proxy Access
    If you are wanting to set up proxy access for a family member etc. you need to have online access yourself. Please ensure that when completing the online access form for proxy that you complete one for yourself if this is required as you can then come into the surgery to confirm your identity in one visit, rather than being asked to return.
    Please be aware that requests for proxy access may be rejected if, for example, a husband and wife each have an online account but are also wanting proxy access for each other.
  • Forms
    You can download the forms required for access at the links below, or you can come and pick up a form from the practice. Please do not complete multiple forms for the same access. We are working hard to get through the thousands of forms that have been submitted since we have changed clinical system. This is very time consuming and can be difficult when patient’s have submitted multiple forms.
    Please be patient with the team. The practice has been unlucky in the number of issues that have arisen with online access, and are unable to get outside help to work through the requests. We are working through these alongside managing our normal roles within the practice.

Thank you for your patience during our clinical system change. We are working through all the online services access queries for those patients who previous had access prior to the change.

If you require online services access and haven’t had this previously, please click below to download and complete the relevant forms. Once completed, please bring the forms into the surgery with relevant ID as stated.

You are able to use a range of apps to access your medical record such as; Airmid, SystmOnline and the NHS App.

Once you receive access you will be able to request a repeat medication 15 days prior to its due date and book available face to face appointments with a GP. If you require an acute prescription (a medication that isn’t on repeat) please complete an e-consultation form via our website. If you require a telephone appointment with a GP or any other type of appointment, e.g. with a Nurse or Phlebotomist, you will need to either complete an e-consultation form or telephone the surgery in the usual way.

If you a parent/guardian or a carer for someone else, you are able to gain proxy access to manage their medication and book face to face GP appointments.
